All About the Annual Wellness Visit

If you get an annual physical, you may wonder why you need an Annual Wellness Visit (AWV). This visit can help prevent health issues from becoming more serious and perhaps even help maintain your independence.

Annual Risk Assessment

Your annual Health Risk Assessment (HRA) helps us identify any health risks you might face and give you personalized recommendations to help you stay healthy. There are two ways to complete your HRA:

  • Visit shop.healthteamadvantage.com/hra to fill out the assessment online.
  • Call your Healthcare Concierge at 888-965-1965 (TTY 711) to complete the HRA over the phone.

Members on the HealthTeam Advantage Diabetes & Heart Care (HMO CSNP) plan are required to complete the HRA annually. It takes about 10-15 minutes to complete.

 

Keep Your Doctor’s Office Informed

It’s important to keep your doctor’s office informed of any changes you experience. Maybe it’s as simple as letting the office know you have moved or had a phone number change. Or maybe you have difficulty stretching your monthly income to cover all your current prescriptions. If so, speak up in the office. The worst thing you could do for yourself would be to walk out with the prescription, thinking there is no way you can ever have it filled! 

 

If you are having trouble getting your prescriptions or basic necessities such as food, there are resources available in the area that can help. Please do not hesitate to speak up at your appointment or call the number on the back of your HealthTeam Advantage card so we can help you access those resources!
